Benefits – Effective 12 months to the day of purchase for:
Adults: 16 & older for $449 , Children: 15 & younger for $299
No Waiting Periods, Deductibles or Annual Maximums!
- 2 regular exams and 2 cleanings each year from the date of the signed and paid contract
- 15% courtesy on all crowns, bridges, implants, fillings, dentures, emergency exams, extractions, whitening and laser treatments, additional cleanings including laser therapy
- All x-rays are included with your cleaning visits except 3-D imaging
